What is Home Health Care?

Who qualifies for home health services?
The answer varies on every patient and situation, but anyone who has a new medical diagnosis, medication or a recent hospitalization may qualify. A home health evaluation will be conducted by a registered nurse to determine if you are eligible. After the evaluation, we will be in touch with you to confirm on a scheduled visit to your home.

How soon can I expect to receive care?
You will receive care starting on the very first visit which comes as soon as you get approved for our services.

We provide care programs such as Skilled Nursing, Rehabilitation Services, Home Health Aide Services, Physical Therapy, Occupational Therapy, Speech Therapy and Medical Social Services.

How do I get home health care?
All home health care is provided under the direct supervision of your physician. To receive home health care your doctor needs to prescribe the care or treatment fit for your needs.
